Use this form to request a return or refund for an eligible Petio order. The system validates your order details and then sends eligible requests to staff review.

Workflow used by this system

  1. Customer submits request.
  2. System validates order, email, order status, refund amount and 30-day window.
  3. Eligible requests become Pending Staff Review.
  4. Staff approves or rejects and adds a review note.
  5. Customer is notified by email where mail service is available.
